Personal contact information provided in a web form is retained so that the practice can get back in contact with you, and may be necessary to provide you with health service.

Information collected includes:

  • Your Phone number;
  • Your Email address; and
  • Your Computer’s IP Address.

Pages on this site may include embedded content (e.g. videos, images, articles, etc.) from other websites. Embedded content behaves in the exact same way as if the visitor has visited the other website.

Definitions

“The Practice” refers to Dr David Taylor Colorectal Surgeon, being the medical practice of Dr David Taylor (MSSB/FRACS CSSANZ).

“Health Service” means information or an opinion about:

  • An activity performed in relation to an individual that is intended or claimed (expressly or otherwise) by the individual or the person performing it:
  • To assess, record, maintain or improve the individual’s health; or
  • To diagnose the individual’s illness or disability; or
  • To treat the individual’s illness or disability or suspected illness or disability; or
  • The dispensing on prescription of a drug or medicinal preparation by a Pharmacist.
